HOW to sign up with our team?
- Establish with a Primary care provider at the VA if you have not already done so.
- Speak with your VA PCP doctor and request a referral and authorization for Chiropractic Care. Be sure to communicate you want intial chiropractic active care and not just chronic pain management.
- Your PCP will submit an authorization request to the VA community Outreach program.
- You will receive a call from a community outreach program coordinator who will ask or direct you to a chiropractor… NEXT STEP IMPORTANT
